Upgrade from 1.0 to 1.1
- Added the ability to toggle validation for all fields.
- Made the confirmation page optional.
- Added the option to use html mail.
- Made the form page editable in wysiwyg editors.

- moved the confirmation table back to the confirm.php file, because it was causing errors in the config file
- Added additional css so you can style everything without having to edit confirm.php.
